You board with your shoes on comfortably. Five hours later they will not go back on properly, your socks have left a ridge above the ankle, and your feet look like they belong to someone else. It is one of the most common complaints in air travel and one of the least explained at the gate.
This piece covers why it happens, how long it should last, the specific signs that mean it is a matter for a doctor rather than a footstool, and what genuinely helps before, during and after a flight. Sources are listed at the end so you can check any of it.
Why flying makes your ankles swell
Three ordinary things stack up, and none of them are about the aircraft being unsafe.
Your calf muscles stop working. This is the big one. The lymphatic system — the network that returns excess fluid from your tissues — has no pump of its own. Unlike blood, which the heart moves, lymph depends largely on the squeeze of surrounding muscle, particularly the calf. Sit still for four hours in a fixed seat and that squeeze simply stops happening. Fluid that would normally be moved back up the leg settles instead. Gravity has an uninterrupted run at you. Your feet sit below the level of your heart for the entire flight, with no walking, no leg-raising and very little position change. Fluid runs downhill all day on the ground too — that is why legs feel heavier by the evening — but on a plane you remove the movement that normally offsets it.
The cabin is dry and pressurised. Cabin air is unusually dry, which nudges people towards mild dehydration over a long flight, and the reduced cabin pressure is generally thought to contribute as well. These are real but secondary. If you fix only the hydration and none of the movement, you will still land with puffy ankles.
Add the usual civilian factors — a salty airport meal, alcohol on board, tight waistbands, warm weather at the other end — and the effect compounds. The NHS lists sitting in the same position too long and eating too much salty food among the everyday causes of swollen ankles, feet and legs (source 1).
How long the swelling should last
Ordinary flight swelling is symmetrical — both ankles, roughly equally — and settles on its own. Most people find it eases within a few hours of walking about normally, and is gone after a night of sleep with the legs horizontal.
A rough guide to what is unremarkable:
- Both ankles and feet, about the same on each side
- Puffy and tight, but not painful
- Skin normal colour, normal temperature
- Noticeably better after a walk and a night lying down
- Gone within 24 to 48 hours
If it is still there on day three, or it never fully settles overnight, that has moved out of the ordinary category.
The signs that are not ordinary
This is the part worth reading properly. Long-haul travel is a recognised risk factor for deep vein thrombosis (DVT), a blood clot in a deep vein, usually in the leg. It is uncommon, but it is serious, and it can start out feeling like ordinary travel swelling.
Get medical help urgently — GP, NHS 111, or A&E — if:
- The swelling is in one leg only, especially the calf or thigh
- There is throbbing, cramping or aching pain in that leg
- The skin is warm, red, darkened or discoloured around the painful area
- The leg is swollen and the veins are unusually prominent
Call 999 or go to A&E if you also have breathlessness, chest pain, or you cough up blood — these can indicate a clot that has travelled to the lungs (source 2).
Risk is higher on flights over about four hours, and higher again if you are pregnant, have recently had surgery, are over 60, take the combined contraceptive pill or HRT, have had a clot before, or have a family history of them. If any of those apply to you, speak to your GP or pharmacist before you fly rather than after.
Separately: swelling that is persistent, in one limb, and long-term can be lymphoedema, which is a recognised medical condition and needs a proper diagnosis rather than a routine (source 3). And if you have known heart, kidney or liver problems, or you have varicose veins that are getting worse, new or increasing swelling is worth a GP appointment (source 4).
What actually helps — before, during and after
Ranked honestly, most effective first.
- Move your calves, often. This is the single highest-value thing you can do and it is free. Every 30 to 60 minutes: 20 ankle circles each way, 20 heel raises with your toes on the floor, 20 toe raises with your heels on the floor. You can do all of it seated without disturbing anyone. It directly replaces the muscle pump you have switched off.
- Walk the cabin roughly once an hour when the seatbelt sign allows. Two minutes down the aisle and back does more than an hour of fidgeting.
- Wear flight socks. Graduated compression socks are the one purchase in this list with a genuine evidence base behind them for reducing travel-related swelling. Get them fitted properly — the wrong size is worse than none. If you have diabetes, peripheral arterial disease or any circulation condition, check with a pharmacist or GP first.
- Drink water and go easy on alcohol. Not because dehydration is the main cause, but because it stacks on top of the main cause, and because it gets you up to use the toilet, which forces the walking.
- Loosen everything. Take your shoes off during the flight (put them back on before landing while you still can), avoid tight socks with a hard elastic band, and do not fly in your most fitted jeans.
- Get your legs up when you land. Ten minutes with your legs up a wall, higher than your heart, does more for the evening after a flight than anything you can buy. Combine it with a walk earlier in the day.
- Watch the airport salt. Departure-lounge food is reliably salty, and the NHS recommends adults have no more than 6g of salt a day (source 5). One meal will not cause the swelling, but it will not help.

What does not help much
- Water tablets. Prescription diuretics are medicines for specific medical conditions, not travel comfort, and taking them without a diagnosis is a genuinely bad idea. We have explained the difference in water pills and herbal drops are not the same thing.
- Restricting fluid. Drinking less does not reduce fluid retention and makes the dry-cabin problem worse.
- Aspirin "for the flight". Do not self-medicate for clot prevention. If you are in a higher-risk group, that is a conversation with your GP, not a decision to make at duty free.
- A single big stretch at the end. Frequency beats intensity here. Twenty ankle circles every hour beats five minutes of stretching once.
Where a supplement fits, honestly
